PM Family Care Tracker and Health Passport pilot launched in Gandhinagar

A pilot project of the PM Family Care Tracker (PM-FCT) and the Health Passport was launched on 28 June 2026 for the Gandhinagar Lok Sabha constituency in Gujarat. The initiative was described as a step towards transparent, technology-driven delivery of welfare benefits, with the stated aim of ensuring that no eligible family is left out of government schemes.

The PM Family Care Tracker is an integrated digital platform that brings together data from several departments. It links records on birth and death registration, health, nutrition, education and social welfare into a single unified system. By connecting these databases through a common identity framework, the platform is meant to give officials a complete picture of each family's needs.

A core feature of the system is automatic alerts. If a child misses a scheduled vaccination or drops out of school, the platform is designed to flag this and send notifications to local officials, legislators and Members of Parliament. The goal is to let government staff and community volunteers step in quickly. The tracker is intended to monitor the health and nutrition of children from the prenatal stage up to the age of 16.

The Health Passport works alongside the tracker as a digital health record for each family. State officials described the combined system as a structured digital health infrastructure aimed at supporting mothers and children from pregnancy through adolescence. The approach was framed around four pillars of healthcare: preventive, promotive, affordable and digital.

During the launch, the first set of digital certificates and Health Passports were issued, polio drops were administered and nutrition kits were distributed. As a pilot, the project is being tested in one constituency first; lessons from this phase are expected to guide any wider rollout of the model in the future.
